spring at Central School, please use the forms on the returning students page

Residency Form- one per family-
must be accompanied by copies of acceptable documents proving residency
pages 1 and 2 are required, pages 3-5 are to be used as
needed and outlined in page 1 of Residency Form

Enrollment Form - one per student

Registration Form - one per family

Emergency Information Form - one per student

Health Information Form - one per student

General Authorization Form - one per student
includes Pictures/Images Permission, Internet Use, PowerSchool Use

Child Health Examination Form - one per student

Dental Examination Form or Dental Exam Waiver Form- one per student
